Common to all

What every record includes.

  • OD/OS laterality in examination, diagnosis and treatment.
  • Complete visual acuity: Snellen, low vision and Jaeger.
  • IOP with method and pachymetry.
  • Refraction and keratometry with add power and interpupillary distance.
  • Dilation timer that doesn't lock the screen.
  • Comparable studies in slider, side by side, flicker and 2×2 grid.

Frequently asked questions

About subspecialties.

Can I select more than one subspecialty?

Yes, and that's normal. Section lists add up: a pediatric neuro-ophthalmologist selects both and gets visual field, optic nerve, gaze positions, binocularity, alignment, motility and sensory function. Selecting a subspecialty never removes sections, it only adds.

What if my practice is general ophthalmology?

The base record already includes chief complaint, vital signs, visual acuity and IOP, anterior segment, fundus, diagnosis, prognosis, plan, prescription, and follow-up, plus the sections NOM-004 requires. You don't need to declare a subspecialty to start working.

Can I change the sections of my note later?

Yes. Sections are added, removed and reordered whenever you want, without programming. Those required by NOM-004 are always kept.
