The overage price is the same on every plan: iGalenus doesn't charge a small practice more for the same unit. The “effective cost” above is something else: it's what each visit in the plan comes to if you use up your allowance. Moving up a plan isn't about paying less per visit, but about adding locations, storage, and reminders. Stamps are charged practically at cost.

No. On the free plan you pay only for the AI you use: when the allowance runs out, the system keeps going and AI is charged by usage, at the published prices. On paid plans you set a monthly cap, the system warns you at 80%, and what you don't use rolls over to the next month.
Nothing stops working: the note is saved, the study is uploaded and the invoice is stamped. Anything beyond the allowance is charged at the per-unit price, or not charged at all if you set a cap and it has been reached.
No. No plan charges for or limits users: physicians, reception, optical, nursing and assistants join at no cost on every plan, including the free one. What changes between plans is the monthly allowance for AI, stamping and storage.
